SG1 Soccer

Please read through each page carefully to ensure that all information is provided accurately and that you understand the terms and conditions.  Once you submit registration, you are authorizing SG1 Soccer to deduct the scheduled payments based on payment schedule that is listed under Payment Options in this registration form.

During the registration process you will be asked to upload the following documents: 1) an electronic copy of your child's birth certificate or his/her passport 2) a CLEAR CURRENT headshot picture of your child.  

Birth Certificate/Passport - Must be clear and readable.  Acceptable documents: US Birth Certificate OR a US Passport   International documents will be accepted however there are times that some will not pass through which may cause a delay in registration.  If any issues arise, our admin staff will notify you.

Headshot - Must be a color photo, taken in the last 6 months.  Use a clear image, no filters.  Use a plain background.  Headshot should be from the top of the head to mid chest.  Center the face in the middle of the picture.  Submit the headshot vertically and not upside down or turned to the side.

Payment types accepted are the following Visa, Mastercard, Discover Card, and ACH E-Check.
